Supplements you were prescribed, simply state the name of the product and indicate big or small bottles if they come in 2 sizes.
The Address you want your supplements shipped to.
Then click the paypal button Buy Now below and enter your:
Total - To get your total, add up all the products from your order form. If you don't have an order form, let us know and we'll total it for you.
+ $13 for shipping & handling (If you order under $200),
or $7 for shipping & handling (If you order over $200)

Once your funds have been received, you will be emailed an invoice for your records.
